CCLHDN MEMBERSHIP: If your county has not paid dues for 2013, you may do so by completing this form and submitting it to the CCLHDN secretary (address on form). Not sure if your county is current on due? Check here for a list of counties who have already paid dues in FY13. Check with your other nutrition program staff in your county (WIC, etc.) to determine if a voting member has already been established. CCLHDN can also help.
You can pay dues via credit card (below) or via check. Only one membership fee per county is paid for multiple staff from that county to attend the conference (each registrant pays individual registration fees, however).
